Robotic Radical Cystectomy with Urinary Reconstruction

  • The most advanced surgical approach for invasive bladder cancer.
  • Performed using robotic-assisted surgery, allowing for precision removal of the bladder while reducing blood loss and enhancing recovery.
  • Urinary reconstruction options include:
    • Neobladder reconstruction – A new bladder is created from the intestine, allowing patients to urinate normally.
    • Ileal conduit urinary diversion – A segment of intestine is used to direct urine to an external stoma.
    • Continent urinary reservoir (Indiana Pouch) – A pouch is created from intestines, allowing catheterized urine drainage.

Transurethral Resection of Bladder Tumor (TURBT) with Blue-Light Cystoscopy

  • A minimally invasive procedure that allows for early-stage tumor removal without major surgery.
  • Blue-light cystoscopy & Narrow Band Imaging (NBI) increase tumor detection rates, reducing recurrence risks.

Intravesical Therapy (BCG, Chemotherapy)

  • Bacillus Calmette-Guerin (BCG) therapy is the gold standard for treating non-muscle invasive bladder cancer.
  • Intravesical chemotherapy may also be used to reduce recurrence and progression rates.

Systemic Therapy for Advanced Bladder Cancer

  • Platinum-based chemotherapy remains the standard for metastatic bladder cancer.
  • Immunotherapy (Checkpoint Inhibitors like Atezolizumab, Pembrolizumab) offers a new approach for chemotherapy-resistant bladder cancer.
